This form is not for public use. It must only be completed by the congregants and affiliates of City Centre Church. The form must be completed for the use of our facilities for events not related to our regular ministry events. Please note that we require 4-6 months minimum notice for use of the facility. Use of the facility is at the discretion of the church office.
